Interoperability Healthcare Design

2016 - 2017

In the consulting industry, each situation is unique and requires a specific set of solutions. For Carlton County, Minnesota - 35,000 lives needed to be covered by an Admission, Discharge and Transfer System. Using a grant, we secured the funding, designed the concept, solicited and won multi-stakeholder buy-in, chose a technology partner and implemented a cross-sector information network to transmit  health information to care teams. Partners included rural clinics (3), critical access hospitals (2), Indian Health Service, mental health, public health and the county, long term care (2) and nursing home (1).

Integrated Health Model

2016

Urban health care for immigrant populations is an underdeveloped part of our health care system. Delivering integrated services that meet and exceed client and staff expectations is a challenge when resources are scarce. For Community Health University Care Clinic integrated services is part of their desired care model. SAMHSA grant funds were used to redesign service delivery for patients with co-occurring conditions. This included workflow, staff procedures, information system and addition of performance metrics to track outcomes.

evaluation of current state

2015 - 2018

Current state evaluation is a necessary component to the discovery process. Knowing how to accurately assess and then demonstrate where a client, collaborative or state is on a continuum of maturity is an art and a science. Using qualitative and quantitative methods we have evaluated the current state of more than 50 clients. Our team has  delivered services to single building organizations and whole state health information exchange ecosystems.
